• Entry fee includes: green fees, cart and lunch
• Make checks payable to FUMC-CCDC (Golf Tournament)
• To be a "Hole Sponsor" return $100.00 fee with information to be printed on "Sponsor Sign" to the address below.

You may pick up an entry form at the CCDC or the FUMC main office.  Please return the form and fees to the CCDC office or the FUMC main office located at 400 East 6th Street or mail it to the address below:
